Exercise your rights as a competent adult to decide who you want to help you, if you become incapacitated. If you fail to choose people you trust to make decisions for you, then a Court will appoint people to act for you.
This document allows you to choose the people (your health care agent) who may give a medical doctor instructions when you are unable to communicate your own instructions. Click here to see a form I use in my practice.
